Under 17 U.S.C. § 512(c)(3): (1) your physical or electronic signature; (2) identification of the copyrighted work claimed to be infringed; (3) the URL and description of the material you want removed; (4) your address, telephone number, and email; (5) a statement that you have a good-faith belief the use is not authorized by the owner, its agent, or the law; and (6) a statement, under penalty of perjury, that the information is accurate and that you are the owner or authorized to act on the owner's behalf.

If your material was removed in error, you may send a counter-notice with your signature, identification of the removed material and its former location, a statement under penalty of perjury that removal resulted from mistake or misidentification, your contact details, and consent to the jurisdiction of the federal court for your district (or the Southern District of Florida if outside the U.S.). We may restore the material in 10–14 business days unless the claimant files a court action.

Knowingly materially misrepresenting infringement or wrongful removal can expose you to damages under 17 U.S.C. § 512(f). We terminate access for repeat infringers in appropriate circumstances.
